Warranty Instrument Repair Instructions
BR Surgical no longer offers repair or sharpening services for surgical instruments other than our Gold Series Biopsy Punches, or if you have a warranty concern for a BR Surgical brand instrument.
Please fill out this form only if you have a BR Gold Series biopsy punch in need of sharpening or have a warranty concern with a BR Surgical brand instrument.
To help us give you prompt service on warranty repairs, please follow the simple steps outlined below.
- Complete this form and click the Send Request button.
- You will be contacted by email with a Repair Authorization Number. Please do not ship without receiving this number first.
- Package the instrument(s) with a copy of the Repair Authorization Number and ship to:

Jacksonville FL 32224 - Once we receive the instrument(s), we will evaluate the instrument to determine if there is a warranty concern. Upon confirmation of the warranty concern, BR Surgical will repair or replace it at our discretion.
Instruments must be sterilized/disinfected before shipping to BR Surgical.
